Chemistree at the Care Show, London Olympia, 11th and 12th October 2011

Chemistree is a specialist provider of pharmacy services to Care Homes in the NHS and Private Sector and will be exhibiting at the Care Show, London Olympia on the 11th and 12th October 2011.

Residents in care homes usually receive support from a range of healthcare services including dentists, district nurses, GPs and pharmacists. Succinct communication between the pharmacist, GP and home can lead to a reduction in poly pharmacy and medication errors in the care home.

The CHUMS report identifies the integral role of the pharmacist between the GP and home stating; ‘Pharmacists should regularly review residents and their medication; they can also rationalise regimes to help home staff work more safely’. In order to comply with this Chemistree have developed Pharmacy Advice Forms (PAFs) to provide advice to GPs and carers about medications. This can reduce poly-pharmacy and improve clinical outcomes for residents, whilst providing a regular monthly review and audit of the drugs being administered.

Chemistree have also pioneered the UK’s first Single Use Medication System [SUMS], an innovative solution to improve the safety and usability of Monitored Dosage Systems for the Care Sector and their service users.
